What you’ll need before you start
Business details
Legal name, DBA (enter the legal name again if they are the same), physical address (no PO boxes), phone, email and nine-digit EIN. FNS number if you accept EBT.
Each owner
Name, ownership percentage, date of birth, home address, cell phone and nine-digit SSN. The checklist mentions 25% only as an example; follow the application questions and list each owner.
Bank document
A voided check, or a bank letter signed by an authorized representative of the bank. It must show both the legal business name and the DBA, plus the routing and account numbers.
Photo ID
A driver license or passport for each listed owner. PDF, JPEG or PNG, up to 8 MB each. Images must be clear, legible and uncropped.
